Signature Aviation Wellness Incentive Program
Welcome to the Signature Aviation Wellness Incentive Program (the “Program”). Signature Aviation USA, LLC. (“Signature Aviation”) is pleased to present the Program to its employees. The Program is a voluntary wellness program made available by Signature Aviation to employees enrolled in certain U.S. medical plans. To learn if the program applies to your plan, contact the Signature Aviation Benefits Service Center at 1-844-260-3601 or contact your local HR Representative.
The Program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the Program you will be given an opportunity to do the following voluntary activities:
- Download a Wellness Screening Form.
- Complete a wellness screening through your own physician or healthcare provider.
- Wellness Screening must include the following:
- Height/Weight or BMI
- Waist Circumference
- Blood Pressure
- Fasting or Non-Fasting Blood Sugar
- Total, LDL, and HDL Cholesterol
- Submit a fully completed and signed Wellness Screening Form.
You are not required to complete any of these activities. However, if you choose to do so and you enroll in a Signature Aviation medical plan, you will receive a premium discount up to the amount of $40 each month. If your spouse is also enrolled in a Signature Aviation medical plan, your spouse must also complete a separate Wellness Screening Form for you to receive a premium discount. You are not required to do any of these activities, but only those employees who do so will receive this incentive.
If you are unable to participate in any of these activities on account of a disability, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard. The information from your wellness screening will be used to provide you with information to help you understand your current health and potential health risks. You also are encouraged to share your results or concerns with your own physician or healthcare provider.

IMPORTANT! Signature Aviation’s Wellness Incentive Program applies to U.S. Medical plans within the Aon Active Health
Exchange. The program excludes plans in San Francisco and Hawaii. This is a plan summary developed for your convenience. All
plans and programs are subject to official plan documents and terms of those will prevail. The Wellness Incentive Program can
